Safety switch issue?

I took my DB15CCMLB out for the first time a few weeks ago. I put about 200 rounds through it. I only had one issue with about 15 rounds. After a round went down range, spent casing ejected, the safety partially (barely) engaged. I had to switch it back to "fire" mode in order to fire the next round. I didn't notice if it was a specific magazine but have since then marked my mags for the next trip to the range. I'm using brand new (as of that trip) Magpul gen 2 magazines. Anyone else experience this or have an idea of what the issue could be?

Try checking the safety detent pin and spring. It should click in place solidly. Could have been inserted backwards.
